I know “kouman ou rele” and “ki jan ou rele” but I’m really bad at remembering names (m toujou bliye yo), so how can I say “remind me what your name is?”

You can say:
Raple m non ou.
M pa sonje non w. Raple mwen kouman ou rele tanpri.

Fè m sonje kouman w rele.
